Support workers required for Hospital Discharge Services.
Services will cover:
In Hospital
Visting
Clean Clothes
Contact with family
Checking clinets property is secure
When Discharged
Inform care providers
Support re-settlement at Home
Home is clean and warm
Food Shopping and Meals
Deal with correspondence and bills
Access support services
Benefits Support - signposting
Telecare Services
Assisted technology
Part time flexible hours to suit, Enhanced CRB required. Full induction will be given. |
